An ACR (Architectural Change Request) form must be submitted and approved by the HOA Architectural Control Committee (ACC) for all new construction and for any modifications or alterations to existing homes or properties that could potentially alter the exterior appearance or features of the property. Please review The Glen Declaration of Covenants, Architectural Guidelines, and Master Landscape Plan documents to determine HOA requirements before proceeding with any exterior improvements/alterations on your property – these documents are available at the www.theglenhudsonwi.com website. Please contact a member of the HOA Board of Directors listed in the website if you have questions or need assistance.
Submit the ACR directly to the board at the bottom of this page and hit the ‘Submit’ button.
Please submit the form and associated documentation as soon as possible and at least two weeks before you plan to begin work to provide time for the ACC to review your request. Wait for a response from the Architectural Control Committee before proceeding with the work outlined in this form. The ACC will contact you if there are questions or clarifications needed. The ACC response will be mailed or emailed to you.
Documentation: Please submit appropriate supporting documentation for this request. The better the information you provide the easier and quicker the ACC will be able to review your request. Below are examples of documentation that can be submitted but this is not an exhaustive list – several of these may be needed:
- Drawings and graphical layouts of the proposed alteration/improvement
- Lot plan with placement of improvements/alterations in relation to lot lines, other buildings, etc
- Pictures of similar improvements/alterations completed on other properties
- Detailed contractor proposal(s) and documentation including blueprints, samples, etc
- List of materials that will be used including colors of materials
- Samples or pictures of materials that will be used
The Owner agrees to the following:
- No work on alterations/improvements may commence until written approval is received from the Architectural Control Committee.
- Alterations/improvements must be completed as represented in this request or as modified by any changes required as a condition of approval.
- The Owner is responsible for obtaining any required building permits and adhering to applicable building codes, ordinances, and regulations.
- The Owner, not the The Glen Home Owners Association, Board of Directors, nor Architectural Control Committee, is responsible for (i) the construction standards and specifications relating to the alterations/improvements and construction work; and (ii) determining whether the alterations/improvements violate any restrictions or requirements imposed by any governmental authority having jurisdiction over the property.
- The Owner shall hold harmless, indemnify and defend the The Glen Home Owners Association and its officers, directors, and agents from and against any expenses, claims, damages, losses or other liabilities, including without limitation, attorney’s fees and costs of litigation incurred by the Association arising out of (i) any part of the alterations/improvements which violates any governmental law, code, ordinance, or regulation; (ii) the adequacy of the plans or specifications submitted by the owner in connection with this application; and (iii) the construction of the alterations/improvements.
